Surgical options include liposuction, which works well when the extra fat is not localized to one area of the breast, or resection, which works best when the area to be removed is concentrated in a specific location. Resection involves physically cutting out the undesirable tissue during surgery, and was not always ideal if the excess fat covered a large area. Liposuction provides an alternative to resection. It works by inserting a tube through a small incision, and suctioning out fat.

Bodybuilders, who sometimes develop the condition as a result of their use of hormone supplements, need to correct this unwanted side effect in order to idealize their physique for bodybuilding competitions. Therefore, they are another group of men that have traditionally received this surgery. In recent years, more has been understood about the prevalence of gynecomastia in other adult men. It is now known to be an issue that affects a wide range of men from many different age groups. As more has been discovered about this condition and the options available, more and more adult males are speaking to their doctors about their concerns, and the population of adult males with gynecomastia now accounts for the majority of reduction surgeries performed.
